Chad Hagens
Chad E. Hagens, 39, Rockford, died on Jan. 24, 2015, at Van Wert County Hospital, Van Wert.
Rites are 11 a.m. Thursday at Miller Funeral Home, west chapel, St. Marys, the Rev. Brooks Fetters, officiating. Burial, with military honors, will follow at Resthaven Memory Gardens.
Calling is 4-8 p.m. Wednesday at the funeral home.
